I received a my heath pays rewards card in the mail about a year ago maybe longer.. recently I was on my insurance (buckeye health insurance) website and it says I have a little bit of money on the card for going to checkups and getting flu shots. They offer a My Health Pays rewards program where you earn money for completing certain activities. They claim you can earn up to $500 dollars, but in their list that breaks down the ways you can earn, it only adds up to $250.
